  1. Complete the Alternate Work Arrangement form below with your supervisor if you are amending your normal work week, working remote full-time or split your time between your home office and 1800 Grant St.
  2. Once complete, submit to System Human Resources at
  3. Both employee and supervisor should retain a copy of this agreement.
  4. Review the Remote Work Handbook.
