Digital ID cards: Easy access to your coverage any time

Whether you can't get enough of your smartphone or you want nothing to do with one, CU gives you options to access digital ID cards when you need them.

Access your plan ID card through your providers:

Anthem (Exclusive, Extended, High Deductible and Medicare)

Physical cards mailed: 

  • Late June for plans beginning July 1.
  • Late December for plans beginning Jan. 1.

Digital cards available:

  • June 20 for plans beginning July 1.
  • Mid-December for plans beginning Jan. 1.

  New members: Get your Member ID

  • If you need your Member ID before receving your physical ID card, contact Anthem Customer Care Services at 1-800-735-6072.
  • You'll need your Member ID to register for an Anthem account.

  Register for an Anthem account 

  1. Go to anthem.com/register.
  2. Enter your Member ID, first name, last name and date of birth.
  3. Following remaining instructions to set your user name and password and create your account.

  Print your ID card

  1. Sign in to your Anthem account.
  2. Under the My Plans tab, select ID Cards.
  3. Click the ID card(s) you would like to print, and select Print at the top of the page.

  Download Sydney Health Mobile app 

With the Sydney Health Mobile App, you can find everything you need to know about your medical, pharmacy, dental, and vision benefits all in one place with My Health Dashboard. After you login, you can also access your ID card in the top right corner of the app.

  iPhone:

  1. Go to the App Store.
  2. Search for Sydney Health.
  3. Tap Get.
  4. The app will download to your phone.

  Android:

  1. Go to the Google Play Store.
  2. Search for Sydney Health.
  3. Tap Install.
  4. The app will download to your phone.

 Don't use a smartphone? 

  • If you are an existing or new member who does NOT have a smartphone, contact Anthem Customer Care at 1-800-735-6072 to request your ID cards by mail or to have a digital copy emailed to you. You can print the digital copy at home or save a digital copy for your records. 
  • You can also register for an Anthem account via computer to print an ID card or save a digital copy for your records.

Kaiser Permanente

Physical cards mailed:

  • Late June for plans beginning July 1.

Digital cards available: July 1


  New members: Get your Health Record Number

  1. Call Member Services at 1-877-883-6698.
  2. When prompted press 6, then 1 and wait for a member services representative.
  3. Members will be prompted three times for a Health Record Number before being forwarded to a representative.
  4. Members must verify their identity by having three forms of identification including name, date of birth, Social Security number and address.

  Register for a Kaiser Permanente account and print a card

  • If you have your Health Record Number, register online at Kaiser's new member website.
  • You can print a ID card at from Kaiser's website. 

  Access a digital membership card

  1. You must have a kp.org account and the latest version of the Kaiser Permanente Mobile app on your smartphone.
  2. Sign-in to the app.
  3. Tap the image of the ID Card () at the top of the app dashboard.

  Download the Kaiser Permanente Mobile app 

 Members can access a digital version of their membership card right from their smartphones using the Kaiser Permanente mobile app. A digital card can be used at any Kaiser location that requires membership information. 

  iPhone:

  1. Go to the App Store.
  2. Search for Kaiser Permanente.
  3. Tap Get.
  4. The app will download to your phone.

  Android:

  1. Go to the Google Play Store.
  2. Search for Kaiser Permanente.
  3. Tap Install.
  4. The app will download to your phone.

  • If you are an existing or new member who does not have a smartphone, contact Kaiser Customer Care at 1-877-883-6698 to request your ID cards by mail or to have a digital copy emailed to you. You can print the digital copy at home or save a digital copy for your records.
  • You can also register for a Kaiser account via computer to print an ID card or save a digital copy for your records.

Delta Dental

Delta Dental of Colorado members do not need physical or digital ID cards to receive care, unless they visit an out-of-network provider or a Delta Dental provider outside of Colorado.

Physical cards mailed:

  • Mid-June for plans beginning July 1.
  • Mid-December for plans beginning Jan. 1.

Digital cards available:

  • Mid-June for plans beginning July 1.
  • Mid-December for plans beginning Jan. 1.

  Register for a Delta Dental account

New members can register using their Social Security number. If a new member does not want to use their SSN to register, contact Delta Dental Member Services to get your Subscriber ID at 1-877-FLOSSCU (1-877-356-7728) or email customer_service@ddpco.com.

To get started, register for a Delta Dental of Colorado account:

  1. Got to deltadentalco.com, and select Sign In/Register.
  2. Select I am a member and press Go.
  3. Select Create an account.
  4. Select I am a member or adult dependent and have coverage with Delta Dental and select Proceed to Step 2.
  5. Enter the required information in the fields provided and click Register.

On deltadentalco.com, you can print member ID cards, check your dental benefits, find a provider, check the status of a claim and more.


  Download the Delta Dental Mobile app 

After registering an account, you can download the Delta Dental Mobile App in your phone's app store. Your login credentials will be the same as your website account.

  2. Search for Delta Dental Mobile.
  3. Tap Get.
  4. The app will download to your phone.

  2. Search for Delta Dental Mobile.
  3. Tap Install.
  4. The app will download to your phone.

  • If you are an existing or new member who does NOT have a smartphone, contact Delta Dental Member Services at 1-800-610-0201 (1-877-FLOSSCU) to request your ID cards by mail or to have a digital copy emailed to you. You can print the digital copy at home or save a digital copy for your records.
  • You can also register for a Delta Dental account via computer to print an ID card or save a digital copy for your records.

CVS Caremark

Physical cards mailed:

  • Late June for plans beginning July 1
  • Late December for plans beginning Jan. 1

Digital cards available:

  • July 1 for plans beginning July 1
  • Jan. 1 for plans beginning Jan. 1

  Register for a CVS Caremark account

  1. Visit Caremark.com. (If you are a new member, you can register starting July 1.)
  2. Register an account using your Member ID. If you do not have your Member ID, you will need to enter your name, date of birth, address, and gender. You will be prompted to answer a few background questions to validate eligibility and complete registration.

  • You can also register an account at Caremark.com via computer to print an ID card or save a digital copy for your records.
