If a vendor requests proof of CU's tax-exempt status in Denver, you may need to provide this form along with the CIty/County of Denver tax exemption letter, the DR5002 - Declaration of Wholesale or Entity Sales Tax Exemption, and the State of Colorado Sales Tax Exemption Certificate.
Note: For events in Denver, the supplier may also ask for the Affidavit of Exempt Event form. This must be completed by the organizational unit for each event, and then signed by the OUC.
