This disclosure survey covers the period July 2025-June 2026. The survey form is open August 1-September 30, 2026.

As a CU Chancellor, an Officer of System Administration, or other System employee as designated, you are asked to use this survey form to submit an annual disclosure of outside financial interests and activities to be reviewed for possible conflicts of interest.

The annual disclosure is required by Regent Policy 8.B.1 Conflict of Interest and Administrative Policy Statements Disclosure of Interests and Conflicts of Interest and Commitment in Research and Teaching. The policies require Officers of the University and Officers of the Administration (and other employees as designated by Officers or as appropriate) to submit an annual disclosure of outside financial interests and activities. Please review these policies before completing this disclosure. 

This disclosure covers the following types of information: outside service, employment, and financial interests; immediate family members' professional services; and, other interests/conflicts of interest.

Definitions

  • The term individual conflict of interest refers to situations in which financial or other personal considerations may conflict, or have the appearance of conflicting, with an employee's professional judgment in exercising any university duty or responsibility in administration, management, instruction, research and other professional activities.
  • The term Institutional conflict of interest refers to situations in which the teaching, research, outreach, administrative, financial, operational or other activities of the university could be compromised because of external financial interests and/or business relationships held by the university as a body corporate or by a university or campus official acting within their authority on behalf of the university or campus, that could bring financial gain to the university, campus, any of its units, and the individuals covered by this policy.
  • The term individual conflict of commitment refers to situations in which outside relationships or activities (such as professional consulting for a fee) conflict, or have the appearance of conflicting, with an employee's commitment to their university duties or responsibilities.
