Employees must sign up with the form below and complete the Mini Gym Waiver and Contract. The Mini Gym is managed by the System Staff Council Health and Wellness Committee and the Office of the President.
Your request for access to use the Mini Gym is sent to the Health and Wellness Committee who will then review the signed Waiver and Contract, then grant you the access code (Please allow 2 business days to process). Please note, you are not permitted to share the access code with any other parties.
