The Capital Asset Management for System Administration (CAMSA) form has been redesigned in the OnBase platform. 

Tax Talk: Sales Tax Exemption Documentation [4]

If a vendor asks you to provide documentation of the University's tax-exempt status, you can find relevant certificates on the OUC's Tax-Exempt Information website.
