Form 1095-Cs available on the employee portal
Form 1095-C is now posted in the employee portal for viewing and download.
This marked the first year the University of Colorado was required to provide Form 1095-Cs to employees. Employee Services mailed all employees their Form 1095-C in March 2016. Now, we have made individual Form 1095-Cs available to employees via the employee portal.
Form 1095-C provides employees information regarding employer-provided health insurance coverage. It specifies the months of health care coverage for employees and eligible dependents, and contains information employees may need to report as part of their annual income taxes.
Per the IRS, employees were not required to submit this form with a 2015 tax filing; however, employees should keep this form with any 2015 tax records.
To access your 1095-C in the portal, follow these steps:
- Select the Benefits and Wellness menu.
- Click on View Form 1095-C. (It can also be found in the My Benefits tile by clicking on the details menu, and selecting View Form 1095-C.)
- Additional authentication will be requested. Once authorized, click on Tax Form to view and download your form.
- If a form has not been issued to you, a message will populate stating that no form is available.
